Waiver Form

All riders must complete the Insurance waiver form in full prior to riding at any Hoofbeats Dressage Club show. Riders who have not submitted a properly signed waiver form prior to their first show of the season will not be permitted to ride.

Junior riders must have the signature of a parent or legal guardian in order to ride. Signatures from Coaches will not be accepted.
