Waiver and Release of Liability Form

All riders must complete the WAIVER AND RELEASE OF LIABILITY form in full prior to riding at any Hoofbeats Dressage Club show. Riders who have not submitted a properly signed WAIVER AND RELEASE OF LIABILITY form prior to their first show of the season will not be permitted to ride.

Junior riders must have the signature of a parent or legal guardian in order to ride. Signatures from Coaches will not be accepted.
